Prostate cancer treated with radiotherapy offers long term survival rates

(dailyRx News) Prostate cancer treatments can include a variety of approaches. Surgery can remove the diseased prostate, while radiation therapy can be used to kill cancer cells.

But what’s the long-term effectiveness of radiotherapy for treating prostate cancer? To find out, researchers tracked 25 years of data.

Nearly three-quarters of prostate cancer patients were still alive 25 years after being treated with radiation therapy, according to a newly published study.

These survival rates are comparable to those achieved by surgically removing the prostate – a procedure called a prostatectomy.

“If you’ve had prostate cancer, 15 years of follow up is recommended.”

Frank Critz, MD, founder and medical director at Radiotherapy Clinics of Georgia (RCOG), and colleagues conducted the study which reviewed 30 years’ worth of data. It’s the longest study of its kind looking at the long-term effects of prostate cancer radiotherapy.

“This study, the longest after irradiation of prostate cancer, confirms using the surgical PSA definition (PSA <0.2 ng/ml) that results from this program are equal to that of radical prostatectomy, thus giving men a choice of treatment, and after 15 year follow up if the PSA is <0.2, late recurrence will be rare,” Dr. Critz said in a news release announcing the study results.

Prostate specific antigen (PSA) levels in the blood are used to predict the presence of prostate cancer.

“I commend Dr. Critz and co-workers for raising the bar and stating that a nadir PSA of <0.2 ng/ml is the standard,” prostate cancer specialist E. David Crawford, MD, told dailyRx News.

For this study, the team followed the outcomes of nearly 4,000 men who been treated for prostate cancer between 1984 and 2000.

A total of 3,546 men had undergone two types of radiotherapy. The treatment began with implanting radioactive beads (I-125 implant), which was then followed by radiation that was given externally.

Technology changed over the course of the study. The first group of men studied received retropubic implants placed through the abdomen – a technique which is no longer used. With technology updates, the newer transperineal template/ultrasound-guided approach was used in 2,875 men. This approach involves placing the implants though the skin between the scrotum and the anus.

This study looked at disease-free survival (DFS) of men at 10-, 15-, 20- and 25-year intervals. Disease-free survival is defined as being alive without any evidence of disease.

Here’s what the study found:

Among men who had received radiation-only therapy, the overall DFS was 75 percent at 10 years, and 73 percent 15, 20 and 25 years after treatment.

Men studied were an average of 65 years of age at the time of treatment.

Men who received the latest treatment technology had a 15-year DFS of 79 percent.

Among men who had prostatectomies between 1984 and 2000, the 10-year DFS was 75 percent and the 15-year DFS was 73 percent.

“Although 10-year DFS are a reasonable measure, this study suggests that at least 15 years of follow-up are needed to fully evaluate the effect of any PC therapy,” the authors concluded.

Furthermore, for men with PSA levels of <0.2 ng/ml, recurrence of prostate cancer after 15 years is rare, the authors wrote.

“This study should be cited when discussing long-term results of therapy with men,” said Dr. Crawford, who is professor of surgery, urology, and radiation oncology, and head of the Section of Urologic Oncology at the University of Colorado Health Sciences Center in Denver.

This study was published in the March issue of The Journal of Urology. No financials were provided.

RCOG is the largest community-based prostate program in the United States. It is an affiliate of Vantage Oncology of Atlanta, Georgia.

Newswise — ATLANTA – The March 2013 issue of The Journal of Urology, the official journal of The American Urological Association, includes a study conducted by four physicians from Radiotherapy Clinics of Georgia (RCOG), a Vantage Oncology affiliate. This study, 25 Year Disease Free Survival Rate after Irradiation of Prostate Cancer Calculated with the Prostate Specific Antigen Definition of Recurrence Used for Radical Prostatectomy, is the first-ever to analyze 25 years of follow-up data after radiation therapy treatment for prostate cancer patients.

Frank Critz, M.D., founder and medical director at RCOG, the largest community-based prostate program in the United States, co-wrote this study along with three other RCOG physicians, James Benton, M.D., Philip Shrake, M.D., and Mark Merlin, M.D. The prostate program at RCOG is the only program of its kind to have gathered and maintained a clinical records database for all patients over a 30-year period in a consistent and comprehensive manner.

“This study, the longest after irradiation of prostate cancer, confirms using the surgical PSA definition (PSA <0.2 ng/ml) that results from this program are equal to that of radical prostatectomy, thus giving men a choice of treatment and after 15 year follow up if the PSA is <0.2, late recurrence will be rare.”

This research study included 3,546 hormone naïve men with prostate cancer treated by I-125 implant followed by external beam radiation. The initial cohort was treated with retropubic implants, and as technology improved, the implant technique was changed to a transperineal template/ultrasound guided approach, which was the technique used in 2,875 of these men. Seventy three percent of men had no evidence of prostate cancer 25 years after this irradiation program. To compare like populations, this study compares patients treated between 1984-2000 to two previous prostatectomy studies which analyze 15 years of follow up data and a disease-free survival (DFS) calculation with a PSA<0.2. The surgical definition is much stricter than the one usually used in radiation therapy studies. Results showed comparable DFS at 10 (75 percent, 77 percent, 77 percent) and 15 years of follow up (73 percent, 68 percent, 75 percent). A sub analysis of patients treated with transperineal implants between 1995 and 2000 showed a 15 year disease-free survival (DFS) of 79 percent.

The analysis includes over 300 men whose prostate cancer recurred who were treated 16-25 years ago. Recurrences were PSA defined and results showed the majority of recurrences occurred within the first five years after treatment and no recurrences were noted15.5-25 years after treatment.

This study concludes that follow up evaluations are necessary for 15 years after treatment to fully understand the outcomes of any treatment for prostate cancer. Unlike a previous study on patients recurring after prostatectomy, this is the first to include interval censoring analysis when looking at time to recurrence. If a man’s PSA is <0.20 ng/ml 15 years post treatment, later recurrence of the disease should be rare.

Frank Critz, M.D. to Present Newest Study Results from RCOG’s ProstRcision® Program Analyzing 25 Years of Radiation Therapy Data for Prostate Cancer Patients

ATLANTA, GA (April 00, 2013) – According to the American Cancer Society, there will be an estimated 238,590 new cases of prostate cancer diagnosed in the US and an estimated 29,720 deaths in 2013. In its continued efforts to increase awareness about the prevention, detection and treatment of prostate cancer, Radiotherapy Clinics of Georgia (RCOG), a Vantage Oncology affiliate, is hosting a free Prostate Cancer Symposium on Saturday, April 20th from 8 a.m. to 10:30 a.m. at the Marriott Evergreen Conference Resort Center, located at 4201 Lakeview Drive in Stone Mountain. The event features RCOG Founder and Medical Director Frank Critz, M.D., who will present attendees the newest study results from its ProstRcision® (seed in-plant followed by radiation) Program, which is the first to analyze 25 years of data after radiation therapy for prostate cancer patients.

RCOG, which has the largest community-based prostate program in the U.S, published its study, 25 Year Disease Free Survival Rate after Irradiation of Prostate Cancer Calculated with the Prostate Specific Antigen Definition of Recurrence Used for Radical Prostatectomy, in the March 2013 issue of The Journal of Urology, the official journal of The American Urological Association. Dr. Critz co-wrote the study along with three fellow RCOG physicians, James Benton, M.D., Philip Shrake, M.D., and Mark Merlin, M.D. RCOG is the only prostate program of its kind to have gathered and maintained a consistent and comprehensive clinical records database of all patients over a 30-year period.

The study followed 3,546 men with prostate cancer, treated with prostate seed implants followed by external beam radiation therapy. None of the men included in this study received pretreatment hormone therapy. The implant procedure used in these patients was updated in 1992 after improvements in treatment techniques. From the sample, 73 percent of patients had no evidence of prostate cancer 25 years after this irradiation program. The study compares patients treated between 1984 and 2000 to two previous studies that looked at patients who had surgical radical prostatectomy. Results of these three studies showed comparable disease-free survival (DFS) at 10 (75 percent, 77 percent and 77 percent) and 15 years of follow-up (73 percent, 68 percent and 75 percent) respectively, showing that prostatectomy and the combination of seed implant followed by external beam radiation therapy are equally effective. A sub analysis of patients treated with transperineal implants and external beam radiation therapy between 1995 and 2000 in the study showed a 15-year disease-free survival of 79 percent.

“This study, the longest after irradiation of prostate cancer, confirms using the surgical PSA definition (PSA <0.2 ng/ml) that results from this program are equal to that of radical prostatectomy, thus giving men a choice of treatment and after 15-year follow up if the PSA is <0.2, late recurrence will be rare,” says Dr. Critz.

A secondary analysis in this publication includes over 300 men treated 16-25 years ago whose prostate cancer recurred after treatment. Recurrences were PSA defined and results showed the majority of recurrences happened within the first five years after treatment and no recurrences 15.5-25 years after treatment. This is the first study to include interval censoring analysis when looking at time to recurrence.

The study concludes that follow-ups are necessary for 15 years after treatment to fully evaluate any treatment for prostate cancer. If a man’s PSA is <0.20 ng/ml 15 years post treatment, later recurrence of the disease should be rare. For more information, visit www.curingprostatecancer.com.

“The Prostate Cancer Symposium is the perfect platform to discuss our study’s results as well as educate attendees about their potential treatment options,” adds Dr. Critz. “Our primary goal for this event is to help men make informed decisions about their prostate health so that they can achieve a positive clinical outcome.”

The event is open to the public and directed towards men recently diagnosed with prostate cancer who are researching their treatment options or have chosen “active surveillance.” Those in attendance will have an opportunity to participate in a question and answer session with Dr. Critz while enjoying a complimentary continental breakfast. For more information about the event, please contact Toni Karasik at Toni.Karasik@VantageOncology.com or call 770-682-2099 ext. 168.

First Study Ever to Analyze 25 years of Data after Radiation Therapy for Prostate Cancer Patients

ATLANTA – The March 2013 issue of The Journal of Urology, the official journal of The American Urological Association, includes a study conducted by four physicians from Radiotherapy Clinics of Georgia (RCOG), a Vantage Oncology affiliate.

The study follows 3,546 men with prostate cancer, treated with prostate seed implants followed by external beam radiation therapy. None of the men included in this study received pretreatment hormone therapy. The implant procedure used in these patients was updated in 1992 given improvements in treatment techniques. From the sample, 73 percent had no evidence of prostate cancer 25 years after this irradiation program. To compare like populations, this study compares patients treated between 1984-2000 to two previous studies that looked at patients who had surgical radical prostatectomy. The 15 years of follow-up data for these three studies calculated disease-free survival (DFS) based on a PSA<0.2 ng/dl. The surgical definition is much stricter than the one usually used in radiation therapy studies. Results of these three studies showed comparable DFS at 10 (75 percent, 77 percent and 77 percent) and 15 years of follow-up (73 percent, 68 percent and 75 percent) respectively, showing that prostatectomy and the combination of seed implant followed by external beam radiation therapy are equally effective. A sub analysis of patients treated with transperineal implants and external beam radiation therapy between 1995 and 2000 in our study showed a 15-year disease-free survival of 79 percent.

A secondary analysis in this publication includes over 300 men treated 16-25 years ago whose prostate cancer recurred after treatment. Recurrences were PSA defined and results showed the majority of recurrences happened within the first five years after treatment and no recurrences 15.5-25 years after treatment. This is the first to include interval censoring analysis when looking at time to recurrence.

This study concludes that follow-ups are necessary for 15 years after treatment to fully evaluate any treatment for prostate cancer. If a man’s PSA is <0.20 ng/ml 15 years post treatment, later recurrence of the disease should be rare.
Prostate cancer is the second leading cause of cancer mortality in men behind lung cancer. It is a disease which affects 1 in 6 men in their lifetime, but it is also one which is almost 100 percent curable when caught early.

The time after you’ve been diagnosed with cancer can be a confusing and frightening process. It is a time when you should try to learn all you can about the disease, and of course, a time when you must weigh all your options and learn about all your treatment choices. Because treatments vary and success rates vary, it is important to find the right treatment for you. Often, many people seek the knowledge and advice of more than one doctor. This is called a second opinion.

A second opinion is the result of a patient or family member who seeks to validate a physician’s recommendation for treatment. Sometimes the second opinion is sought on the advice and recommendation of the first physician, and sometimes the opinion is sought without the first physician’s awareness. In either case, the patient or family member is usually seeking a different perspective for a variety of reasons or attempting to validate the first opinion.

Asking for a second opinion is common practice and most insurance companies will cover the cost; however, it is recommended that you verify with your provider first. Some prostate cancers can be very aggressive and present men with a significant health challenge. The more knowledge you have about your particular diagnosis and the treatment options available, the more comfortable you will be regarding the health care decisions you make. Second opinions lead to more treatment options and give you comfort, peace of mind and a sense of assurance. It is important that you feel confident you’ve made the best decision possible with the best available information before deciding on treatment. Never hesitate to know your options.

Radiotherapy Clinics of Georgia (RCOG), a Vantage Oncology affiliate, has earned a reputation for being one of the country’s top cancer treatment and research facilities. Thousands of individuals from across the country come to Radiotherapy Clinics of Georgia to seek a second opinion and learn more about their treatment options following diagnosis.

With a commitment to quality, use of the latest radiation technologies and research focus, RCOG is committed to providing high quality and compassionate care. Radiotherapy Clinics of Georgia is also the exclusive provider of ProstRcision®, a unique technique for curing prostate cancer that uses a combination of pinpoint irradiation through seed implantation and image guided external beam irradiation. ProstRcision is one of the most reliable and proven modalities in treating prostate cancers, evident by one of the highest cure rates in the world in the over 14,000 men treated over the last 30 years within the RCOG system.

ATLANTA, Ga., April 23, 2012 – The Georgia Prostate Cancer Coalition (GPCC) is pleased to announce that the state of Georgia has approved the issuance of a prostate cancer awareness license plate. Georgia Governor Nathan Deal recently signed the legislation in a formal ceremony in his office. Identification of key GPCC representatives in it appears at the end of this message.

The significance of this legislation is twofold. First, the license plates should be available for purchase by September 2012. Their presence acts as a rolling awareness message on each car with that plate. In addition, the sale of these plates allows GPCC to receive a portion of renewal monies. Those funds will primarily be directed to fund prostate screening for men in Georgia who are either unemployed or uninsured.

“This has been a long time coming and I am pleased and gratified that GPCC has received support both from our membership to help us get qualified and from the State Legislature to pass this legislation,” said Frank Catroneo, Treasurer of GPCC. Catroneo was the initiator and project manager of the effort that raised $25,000.00 and 1,000 signatures required by the State of Georgia to issue the plate. He added, “We had many partners and among our greatest supporters was Radiotherapy Clinics of Georgia (RCOG), a Vantage Oncology affiliate. They were very generous with both organizational and financial support of GPCC from charity golf events and motorcycle rides.”

Added John Whetstone, “While this recognition occurred during my administration, I am grateful for the efforts of prior GPCC leadership that allowed us to reach this important occasion. Certainly individual donors, fund raising proceeds, and support from generous partners helped us tremendously. We could not have done it without them.” Whetstone is President of GPCC.

On a per capita basis, prostate cancer detection in Georgia is 25th lowest in the U.S. while deaths attributed to it are 6th highest. The American Cancer Society reports that prostate cancer is the second most lethal cancer in men and that resulting deaths attributable to it approximate the number of women’s deaths due to breast cancer.

The photo taken in the Governor’s office by his photographer Alana Joyner shows from left GPCC board members Ken Stevens, John Whetstone, and Georgia Rep. Virgil Fludd. In the center is Governor Deal (seated), and third from right is Georgia Rep. Stephen Allison, author of the bill.

About GPCC

GPCC is a 501 (c) (3) non-profit organization whose mission is to increase prostate cancer awareness, advocacy and information exchange. Visit www.georgiapcc.org.

The Centers for Disease Control reports prostate cancer is the most common cancer among men, and one in six men will be diagnosed with prostate cancer in their lifetime, according to the American Cancer Society.

“Prostate cancer affects all men regardless of race or income,” said Knowles. “We have a strong male audience which attends our Rodeo Tour events so it’s a good opportunity for us to educate men and their families on prostate cancer and general men’s health.”

EDITOR:

Prostate cancer is the second leading cause of cancer mortality in men behind lung cancer. The prostate-specific antigen (PSA) blood test, even with limitations, is the best screening method we have today along with the DRE (digital rectal exam). Recently, the U.S. Preventive Services Task Force, a panel of non-urological/oncological experts who review medical evidence for the government, published an open draft recommendation indicating men should stop routine PSA blood tests, which help to screen for prostate cancer. It is my opinion that such a carte blanche recommendation is both regressive and unfortunate.

Notwithstanding the aggressiveness or non-aggressiveness of prostate cancer, men should not be denied the knowledge of knowing a malignancy may reside within them. With such knowledge, each man can make an informed decision as to how to manage his condition. This option to choose should not be dictated by a 16-member, government-sponsored panel. The patient should make such decision in the comfort of his home, surrounded by love ones.

My response to this drafted recommendation from the task force is in the wake of seeing a 40-year-old asymptomatic African-American male in my office last week with an aggressive prostate cancer. He presented to an astute primary care physician who ordered a routine PSA, which was 5.5. If this gentleman does not receive prompt therapy, it is highly likely he will die of his disease. How would your proposed recommendations help this young man delay death from his cancer?

ATLANTA, Nov. 9, 2011 /PRNewswire-USNewswire/ — Prostate cancer is the second leading cause of cancer mortality in men behind lung cancer. The PSA (prostate-specific antigen) blood test, even with limitations, is the best screening method we have today along with the DRE (digital rectal exam). Recently, the U.S. Preventive Services Task Force (USPSTF), a panel of non Urological/Oncological experts who review medical evidence for the government, published an open draft recommendation indicating men should stop routine PSA blood tests which help to screen for prostate cancer. It is my opinion that such a carte blanche recommendation is both regressive and unfortunate.

Notwithstanding the aggressiveness or non aggressiveness of prostate cancer, men should not be denied the knowledge of knowing a malignancy may reside within them. With such knowledge, each man can make an informed decision as to how to manage his condition. This option to choose should not be dictated by a 16 member government-sponsored panel. The patient should make such decision in the comfort of his home, surrounded by loved ones.

My response to this draft recommendation from the USPSTF is in the wake of recently seeing a 40 Y/O asymptomatic African American male in my office last week with an aggressive prostate cancer. He presented to an astute primary care physician who ordered a routine PSA which was 5.5. If this gentleman does not receive prompt therapy, it is highly likely he will die of his disease. How would the proposed recommendations help this young man delay death from his cancer?

Snellville (Nov 1, 2011) – Cancer patients in the Loganville and Grayson area have a new larger radiotherapy facility close by in Snellville to help them with their treatment needs. The Snellville location of Radiotherapy Clinics of Georgia recently had a grand opening to showcase its new building.

“We may be expanding to a new building, but it will be the same great care. Just with new state-of-the-art technology,” said John Gargus, M.D., medical director of Radiotherapy Clinics of Georgia – Snellville.

The new facility incorporates TrueBeam Technology, an FDA approved radiotherapy technology that members of the clinic say allows doctors to optimize treatments using incredible accuracy. The clinic has also decided to revert to its original name.

“Returning to the name ‘Radiotherapy Clinics of Georgia’ reinforces our dedication to providing quality radiation therapy to those suffering with cancer,” said Frank Critz, M.D., founder and medical director of Radiotherapy Clinics of Georgia. “While our name has changed, our commitment to serving the needs of our local communities remains the same.”

Gargus will continue to lead the new facility as its resident radiation oncologist. He was previously a radiation oncologist with Radiotherapy Clinics of Georgia Lawrenceville and rejoined the organization in June as medical director for the Snellville sites. Gargus has practiced radiation oncology in Gwinnett County for the past 15 years.

With five locations in Georgia including the new Snellville facility, Radiotherapy Centers of Georgia is convenient and close to home. To learn more about Radiotherapy Centers of Georgia, or to learn more about the new Snellville location, visit http://www.rcog.com/.

The Down N’ Dirty Rodeo Tour encouraged men to “know their numbers” as they strapped on their boots and grabbed their saddles when the inaugural rodeo commenced in Atlanta on July 9 -10 at the Jim Miller Park in Marietta, Ga. Radiotherapy Clinics of Georgia, a Vantage Oncology affiliate, Xfinity and Omega Psi Phi Fraternity, Inc. served as proud partners of the Down N’ Dirty “Know Your Numbers” Rodeo Tour in support of educating men and their families about prostate cancer. Proceeds from the two-day rodeo benefitted the Georgia Prostate Cancer Coalition and the Comprehensive Men’s Health Initiative.

“We were thrilled to be working with the Down N’ Dirty Rodeo Tour as they promoted prostate cancer awareness in Georgia,” said Shelly Glenn, of Radiotherapy Clinics of Georgia. “This rodeo brought together community members and spread the important message that education and awareness is a key part of prevention.”

The Down N’ Dirty “know your numbers” Rodeo Tour, coordinated by Stanley Knowles, president and CEO of the Down N’ Dirty Rodeo Tour, was committed to increasing awareness in the community about prostate cancer, and welcomed over 1,000 spectators at the inaugural event.

“Prostate cancer doesn’t care how much money you have or the color of your skin,” said Knowles. “It is destroying families, so men – put your pride on the shelf and go get checked.”

The Centers for Disease Control and Prevention reports prostate cancer is the most common cancer among men, and one in six men will be diagnosed with prostate cancer in their lifetime, according to the American Cancer Society.

Information on prostate cancer prevention, screening and quality men’s health were provided to all attendees at the rodeo.

The Down N’ Dirty Rodeo tour proved to be a fun event for all ages as local civic organizations like the Business Development Initiative, Inc. (BDI), a collaborative initiative between the public, private and nonprofit and faith organizations to answer the need for youth to become entrepreneurs and develop ways to train them, proudly accompanied more than 20 of its youth members to enjoy the two-day rodeo event that featured calf roping, barrel racing, steer wrestling, team roping, bareback riding and the most dangerous sport in the world: bull riding.

She is a clinician in the field of behavioral medicine, but it is her own experience with life-altering and catastrophic illness that anchors the subject in an intimate way. Her recent book is acclaimed by Harvard scientists, physicians and epidemiologists as “the Health book to read this year.”
